As we transition back to face-to-face learning, it is important for the College is made aware which students will require continued learning support at home due to a medical condition or for reasons where the student is immunocompromised. If this applies to your child please complete this form.

We are asking for this information now so that those affected families have the time to seek a medical certificate. The College can then register affected students as an "explained absence" (in accordance with the Education Act, 2017), and provide alternative learning options for your child from 9th June.

coronary heart disease

congestive cardiac failure

chronic lung disease such as severe asthma, cystic fibrosis, bronchiectasis,

poorly controlled diabetes

Leukemia, common lymphomas, myelodysplastic syndrome

post-transplant if you have had a solid organ transplant and are on immunosuppressive therapy

post-transplant, if you have had a haematopoietic stem cell transplant in the last 24 months or are on treatment for graft versus host disease (GVHD)

by primary or acquired immunodeficiency including HIV infection

by having chemotherapy or radiotherapy

taking any biological disease-modifying anti-rheumatic drug (bDMARD)

taking any immunosuppressive drugs
