LVMS Music Program Enrollment Form
Please fill out the form below if you wish to enroll your child to participate in the music program at Long Valley Middle School.  

This form should be completed by July 31, 2022 so that we can begin the next phase of our scheduling process here at LVMS.

Student's Primary Instrument
All Music Students must take at least one "quint" (36 days) of Rehearsal.  It is preferred that they participate in 2 "quints" of Music Rehearsal so they can prepare for their Winter and Spring Concert performances.  (Note: Morning/Before School Rehearsals are also part of participation in our Music Programs.)
