2022-23 'Ohana Athletics Senior Elite Tryout Form
We are excited for Season 4 of the team that started it all - Momilani Senior Elite 4.2!

Senior Elite - Birthdates 06/01/2003 - 12/31/2007
*Level 2 tumbling required.
*Confirmed National Travel Team, with confirmed end of season travel to an out-of-state destination
*Tryouts are required for placement onto Momilani.  Not all athletes that try out may be offered placement.
*Summer practices are Tuesdays & Thursdays from 6:30 - 8:30pm.
*School Year practices are Tuesdays & Thursdays from 6:30 - 8:30pm and Sundays from 1 - 4:30pm.
*Monthly tuition is $100/month.  

Please complete the form below in its entirety.  An invoice for the nonrefundable $20 tryout fee will be emailed to Parent/Guardian 1 within 48 hours of submitting this form.  

Tryouts will be held Sunday May 22nd by appointment.  Tryouts will take approximately 1 hour, and athletes should arrive 15 minutes prior to their tryout time to check-in and warm-up.  Tryout times will be confirmed when your payment has been received.

Candidates are encouraged to also complete the 2022-23 Registration Form at https://forms.gle/BAyKbEG4zhSqjGc16    If placement is offered after tryouts, candidate will receive any registration discounts based upon their registration date.
