Please use this form to update the Blaze staff on your uniform needs.

All families MUST fill this form out even if you do not need to order a uniform this season.

3rd - 7th grade athletes will be suited up in our classic style uniforms.
8th - 11th grade athletes will be suited up in the modern style.

We will provide custom sublimated uniforms for athletes:
1) New to Blaze
2) Needing a Larger Size
3) Transitioning from 7th grade (classic) to 8th grade (modern)

ANY CHANGES NEED TO BE MADE BY October 26th after fall tryouts and January 18th for any new athletes after winter tryouts.  We appreciate your help in meeting our uniform deadlines. Any athlete who does not order their uniform by these deadlines will not have their uniform by the first tournament.
