STERLING ACRES KENNEL, LLC policy is to have, on file, a Protocol for our senior guests and/or guests with medical or physical conditions that could be life-threatening. This is a precautionary step to assist us in following your wishes in the unlikely event your beloved pet passes away while in our care.

I authorize STERLING ACRES KENNEL, LLC to transport my pet for cremation and wish to have the ashes returned. This would be a private cremation.

I authorize STERLING ACRES KENNEL, LLC to transport my pet for cremation and DO NOT wish to have my pet’s ashes returned. This would be a group cremation.

I do not wish to have my pet cremated and would prefer to have my pet returned to me for a private burial. I agree that STERLING ACRES KENNEL, LLC will make the appropriate arrangements and I will pick my pet up when I return from my trip.
