The Check-In Team serves on Thursday, November 19th, 8:30 a.m.-10 p.m. (with breaks for lunch
and dinner) and Friday, November 20th, 9 a.m.-2 p.m. This team is also required to serve on
Sunday, November 22nd, for tear-down, Noon-5:00 p.m. (lunch provided).

You agree to cover travel expenses to/from NYWC (including flights, fuel, parking, taxi, shuttle, etc.).

You agree to cover your own lodging expenses.

You will arrive and check in by Thursday, November 19th, prior to 8 AM to be ready for training at 9 AM.

You will finish your Check-In Team role by 1 PM on Friday, November 20th.

You will assist with Convention tear-down on Sunday, November 22nd, from 12 PM to 5:30 PM.

You are willing to offer references and undergo a background check.

In consideration of being allowed to volunteer my services at the National Youth Workers Convention, I hereby acknowledge that there are certain risks of injury involved, and I knowingly and freely assume all such risks and assume full responsibility for my participation with the submission of this form.

Videos, photographs, and audio clips are taken at all conferences to be used for future promotional material.

By submitting this form, I am giving permission to Youth Specialties to use my picture or video in promotional materials.
