[2021 Summer] HYG Covid-19 Survey
Hi!

Could you please fill out the following survey to the best of your knowledge?

As the state of Illinois is now in Phase 5 of the COVID-19 reopening plan, the following are the governmental guidelines:
- no more capacity limits for both indoor and outdoor gatherings (HYG had a self-imposed 25 person limit for Sunday services)
- masks are no longer required, IF you are fully vaccinated (however in public transport, healthcare settings, and in some educational settings masks/face coverings are still required)
- social distancing is no longer required

As things start opening up and get back to pre-COVID conditions, HYG will always put you and your family's safety as a top priority as we plan and hope to gather more intimately this summer.
