My child is enrolled as a student at North Hills Christian School (NHCS) for the 2022-2023 school year and will be participating in school programs and activities related to their enrollment at NHCS. I hereby affirm that I am the parent or legal guardian of Child and I hereby expressly grant my permission for Child to participate in all school programs and activities. I grant this permission with an understanding of the risks that may be related to COVID-19 and Child’s participation in programs and activities as a student at NHCS.

I understand that NHCS cannot eliminate all risks which may be encountered as a result of my child attending the school and/or participating in any school-sponsored program or activities. I realize there are conditions and hazards which independently or in combination with any school program or activity may result in the exposure to certain risks including exposure to coronavirus (COVID-19), or other biological agents, viruses or similar bacteriological agents, and the risk of being quarantined, or illnesses that may result in medical care, hospitalization or death.

I hereby agree not to enter the campus of NHCS nor permit Child or any other member of my family, household or anyone else who may be entrusted to my care to enter the campus of NHCS if I or they have exhibited within the previous 24 hours any of the symptoms then listed by the Center for Disease Control and Prevention as symptoms of Coronavirus.1 If I have any questions or concerns about my child’s health, I may consult with a medical care provider and I am not relying on any information provided by NHCS in any decision regarding my Child’s health.

I understand the risks related to COVID-19 and I understand that I or my Child may be exposed to these risks as a result of our participation in activities and programs of NHCS. To the extent that I do not understand these risks, I have had an opportunity to consult with medical care providers and other professionals who may be able to counsel me on these matters.

I am aware of the COVID19-related safety measures and precautions taken by NHCS to protect the students, faculty, staff and visitors and I hereby acknowledge the reasonableness of those safety measures and precautions. Based on the foregoing, and as consideration for Child’s participation as a NHCS student, I on behalf of myself and Child, do hereby release North Hills Christian School, its directors, employees, volunteers, agents, and representatives from all liability for any loss or damage, and any claim for damages resulting therefrom, on account of any injury, illness or exposure to and/or contracting the coronavirus (COVID-19) or other biological agents, virus or similar bacteriological agent. I further agree to indemnify and hold harmless NHCS, its directors, employees, volunteers, agents and representatives for any liability, loss, damages, costs (including but not limited to attorney’s fees) or settlement which may be brought or entered against them arising out of the negligence, wantonness, or willfulness of the undersigned parent or guardian or any third party and related to Child’s participation in any school programs and activities.

I fully understand, on behalf of myself and Child, the risks associated with the participation in any school programs and activities and assume any risk associated therewith.

I expressly agree that the terms of this release and indemnity agreement are intended to be as broad and inclusive as permitted by applicable laws and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
