The information provided in this form will be used to book your place on the above workshop. You will not be contacted regarding further events unless you specifically ask for that to be the case.
Your full name and address will be used to send out your CPD certificate only.
The purpose of this form is to provide you with the information you will need to pay your fee for the day and to reserve your place. The booking form, and your information, will be deleted once the event has taken place and you have received your certificate.
