I have watched the entire React Team Volunteer Manual Training Video (above), and I agree with its policies and will abide by all of its policies. I, the undersigned, hereby release Faith Church from any and all liability, claims, and demands which may arise from the services I provide to the Faith Church Reflectors Ministry. I understand and acknowledge that this Waiver releases Faith Church from any liability or claim that I may have against Faith Church with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services I provide to Faith Church or occurring while I am providing volunteer services. I understand and acknowledge that I am responsible for my own insurance coverage, including automobile coverage if using a vehicle, in the event of personal injury or illness as a result of my services to Faith Church. Further, I understand that Faith Church does not assume any responsibility for or obligation to provide me with financial or other assistance. I expressly waive any such claim for compensation or liability on the part of Faith Church beyond what may be offered freely in the event of injury or medical expenses incurred by me. *
