Any person or entity who wishes to use materials produced by Health Intervention and Technology Assessment Program (HITAP) shall submit this Material Request Form (MRF) for formal approval for the release of materials. The recipient shall acknowledge that HITAP own the materials, which were originally produced for a training course called “ Sifting for Gold: Systematic Review of Economic Evaluations Training”. Therefore, the recipient shall understand that HITAP are the sole proprietors of the materials with exclusive intellectual, copyrights, and other proprietary rights thereof.

The recipient is required to thoroughly read the attached agreement and sign. It delineates expectations and requirements surrounding the circulation and use of the materials in detail for the recipient.
