Purpose: This Google form is provided to a continuing college student who would like to shift to another course effective this second semester, AY 2021-2022.

By filling out this form, the student certifies that the information provided below is true and correct. All shifting applications will be processed within 2-3 working days. The email correspondence between the student and the program's Dean or office are official and binding.
