2020/2021 Faith Formation Registration

This form allows you to register your student(s) for the programs that are currently available and others that may become available during the 2020/2021 program year. At this time, we will not be able to offer programming that includes group singing in our building, which limits our children and youth choir programs. However, we know that things can change, and hope there will be opportunities later in the year to offer Grace Notes, Nova Choir, and Shalom Choir safely again. We also realize that this may not be possible until much later. Please note on this form if your child(ren) would be interested in participating in those programs if they are offered at a later date.
