This form is for single site registrations. To purchase the program for more than one site, you’ll need to complete the form multiple times OR contact us for manual bulk purchases.

The $500 fee allows for unlimited use of the Check In program for one year at a single implementation site (i.e. individual school, not a school district or individual workplace branch, not entire corporation).
