Use this form to sign up for Evergreen's Weight Loss Challenge!  In this challenge, participants will compete against one another to largest percentage of weight loss throughout a 8 weeks. Participants would first be required to pay an entry fee of $5 to compete. Once paid, participants will be contacted, and will be asked to log their current weight. Every following Friday, participants will be required to log their weight with the corresponding week of the competition. After the final week, May 21st, the percentage of weight loss for everyone will be calculated by the Evergreen team and will announce the top three winners at Forge On 2021.
