Pacific School carpool/bus chaperone interest form
This form is for families who don't live in Davenport, to make getting kids to and from school more efficient. One of us parents tries to make sense of the info this form gathers, please be patient and kind - if it works, it's heaven and worth this clunky sorting-out period! It often takes until September before carpooling is up and running smoothly, not too long! 

Morning bus: Many kids ride the city bus from Mission Street on the Westside. Parents rotate chaperoning the ride.

Afternoon carpool: You drive a carful of kids one afternoon a week, and other parents bring your kid back the rest of the days.

Be aware this form collects addresses and cell phone numbers because they are needed for carpooling. The parent who is going through the results will see this info, and so will the parents who are part of your specific carpool when it gets organized, or who chaperone the bus.

Most kids get out at 1:10 on Wednesdays, answer for the other four days of the week.

You will probably need to send your kid to school with their booster for most carpool days, check with the individual drivers as many have some in their car already.

This is used often to text when carpool is running late. List everyone who will be waiting for your kid - dad, grandma, the babysitter.

If you are meeting at a central place, like Garfield Park Library, include that address. If your kids go to different addresses on different days of the week, list the day and the address for each.
