MathFinder Activity Report for Summer Camp preK - 3rd Grade
Completion of this form is required at the end of each summer camp session, for each group of students in grades preK-3. For example, if your summer camp hosts a new group of students every week, you must complete this form at the end of each of those weeks.  
For Dallas ISD Extended Learning site coordinators, you will complete this form for each group of students every 2 weeks.  

NOTE:  there is a minimum weekly requirement that all students receive 70 minutes contact time with your choice of developmentally appropriate MathFinder activities. 
Remember, you can combine math with play, nature, literacy, and more through this initiative!  Make it fun and engaging for your students.
