BACK 2 SCHOOL SPONSORSHIP FORM
Partner with Berea City of Hope for our upcoming Back 2 School Giveaway being held on Saturday August 19, 2023. With just $9 per child, we will be able to provide students with backpacks full of supplies.

Donations are being accepted through August 15, 2023. Please provide details of your donations below. A link to submit your payment will be provided on the confirmation page of this form.

A $9 donation will provide a backpack full of supplies for 1 child. You can sponsor as many children as you would like. (ex. A $27 donation will provide 3 children with a backpack full of supplies or a $45 donation will provide 5 children with a backpack full of supplies).

All physical school supplies can be turned in to Sis. Jessica Greene during our services time of Tuesdays at 715pm and Sundays at 1030am. All other submission times must be coordinated with Sis. Jessica Greene by email at Jgreene.bch@gmail.com
