Homework Club Permission Form (should be completed by a parent/guardian)
Please complete the following form prior to your student's arrival at Homework Club.  We meet on Tuesdays and Thursdays after school, from 2:30-4:00 in Mr. Steinke's Classroom (room A305). Homework Club is a space for students to review for tests/quizzes, work quietly on their work, meet with groups or partners, or, when available, receive tutoring from students who volunteer their time.

By checking the box below, you are granting permission for your student to stay for Homework Club, which runs on Tuesday and Thursday, from 2:30-4pm. You are also verifying that the student has a way to leave school after the Club meeting(s) that is safe and appropriate. Many students come but do not spend the entire time - this form verifies that you have discussed a time with your student to pick them up, if necessary. 

Due to the nature of this club, and in the expectation of irregular attendance from Club members, this form only needs to be completed once, prior to your student coming to their first session of Homework Club, and applies to all future session attended by this student. 

This form needs to be filled out for each student. If they are meeting with a group - all group members need permission to stay.
