2024 SKZ KUNGFU ENRICHMENT SUMMER CAMP

2024 Shaolin Kungfu Zen Academy Kungfu Summer Enrichment Camp is fun-filled with fast-building sessions. Our summer camp program focuses on building confidence through fun and fitness. It is open to anybody (any level student) who wants to broaden their skills.

Students Should:

1.Bring their favorite books for their own leisure time in the camp when waiting for parents to pick them up.

2.Bring their own lunches & Water bottles. (Staff and TA’s will help heat up their meals.)

3.We will follow the government epidemic prevention guidelines.

All camp weeks run Monday through Friday 9:00am to 3:00pm

3:00 - 3:15 pm Dismissal / Pick Up (Read on your own as you wait).

It is parent and/or guardian's responsibility to pick up their child and/or children on time after Camp hours. 

I (and my child) agree to faithfully comply with all rules and regulations set by this establishment and its instructors. 

I (and my child) also agree to comply with and respect the traditions and cultural practices of the Camp Program. Failure to follow these clauses may result in expulsion of the child. In consideration of being permitted to participate in the Summer Camp, Kungfu classes, its programs or workshops l agree to hold all instructors, and USA SHAOLIN KUNGFU ZEN exempt from any and all damages and injuries during classes and performances at all times. I hereby knowingly and voluntarily assume all risk of injury on my child's behalf while he/she is participating in any programs. I understand that it is my responsibility to consult with a physician prior to permitting my child's participation in martial class classes, programs, and workshops. 

I acknowledge and sanction that my child is physically fit, and my child has no medical condition or underlying medical conditions, which would prevent his/her full participation in exercise classes, programs or workshops. 
              
I give permission to have my child photographed and video-recorded during the course of the Kungfu Enrichment Camp Program. I understand the at such material will be used for educational, outreach, and promotional purposes and waive any rights of ownership.

I have read the above release form and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
