First and Last Name, birthdate, gender, graduation year, ethnicity, email, home address, and home phone number. This information will be used solely for registration for Skills USA.

Once you have completed the registration, please make sure to submit your $20 payment to the main office for the annual dues to Skills USA, our professional student organization. Dues for region ($4), dues for state ($8), and dues for national Skills USA ($8). Upon payment you will receive two receipts, one for the instructor and one for you.
