In accordance with athletic contest requirements, and in order to help protect the health and wellness of all, the morning of the event, and prior to arriving, we require all spectators and competitors to complete a wellness check which includes submitting answers electronically to the questions listed below.  You may submit responses on one form for all members of your household that have the same answers.  There will be an opportunity at the end of the form to enter the names of your additional household members.  If the answer to any of the questions is yes, you are agreeing to not attend our event.

Please be reminded of the following additional requirements for event attendance:
1.  Temperature checks of all spectators will occur at the time of their arrival.
2. All spectators must wear a multi-layer face mask at all times, even when seated with their household and socially distanced from others.  The only exception will be for those with a medical exemption documented by a medical doctor on official letterhead from the office issuing the exemption  Those who refuse to wear a mask for any other reason will not be allowed entry or will be asked to leave the event.
3. Members of the same household must sit together in order to conserve seating areas for others.
4. All spectators must remain at least 6' from spectators outside their immediate household when seated and make every attempt to do so when moving.
5. Spectators must avoid congreting with others before, during and after contests.
6. All spectators under 18 years of age must be accompanied by an adult.
