The Mastermind is a small, intimate group of highly motivated private practice owners who want to scale up their business to a half a million and beyond as well as have the opportunity to network with other highly successful experts.

IMPORTANT: Read this before you apply to the Mastermind.

This application will take 5 - 7 minutes.

We are very selective with who we allow participate in the Mastermind. There is no guarantee that you will be accepted by completing this application.

Here's some of the criteria we use:

1. You're currently earning or "on-pace" to earn $250,000+ in annual revenue. 2. You're currently running a rehabilitation-based private practice business.

If your application is accepted, you will then be asked to have a 20 min application call with Alex Earl. We will reach out to you with information on the next steps and scheduling your call.
