Please complete this form to reserve a table at the WUSD all staff health and wellness vendor opportunity.  An 8 foot table and 2 chairs are provided per vendor.  No electrical support is available. Reservations accepted on a first come-first serve basis with a maximum of 29 vendors for the event.  Please note the district is accepting reservations from local businesses and firms  offering support in physical, mental and financial health and wellness only.
