School District of Milton K-3 Device Agreement for Parents and Guardians
Dear Parent or Guardian:
The School District of Milton is proud to offer families the use of district equipment to assist in the learning process during the extended school closure mandate in spring and out into the summer of 2020. This form authorizes the School District of Milton to allow your student to check out district owned equipment. There is an expectation that the devices will be cared for and returned in the same condition as initially received.
*Please carefully fill out one copy of this form PER student prior to traveling to your school to pick up any devices. For example, if you will be collecting two devices for two students in your household, please fill this form out twice.*
