Bicentennial Recognition & Awards Program: Nomination Form
The Bicentennial Recognition and Awards Committee would like to affirm the outstanding ministry leaders, volunteers, and staff for the exceptional works and remarkable supports they have shown for the mission of St. John Parish and/or School. We are seeking your nominations for individuals who have provided distinguished and outstanding leadership, services, ministry, or loyalty to St. John the Evangelist Church for the mission of St. John parish and school.

Please submit your completed form by next weekend, March 26/27, 2022, so that the Recognition and Awards Committee can verify, discern, and select the nominees. Your nomination must articulate in writing the noteworthy results of the nominees outstanding leadership, outstanding service, ministry achievement, or loyalty to the mission of St. John. Selection of awardees shall not be determined according to the number of nominations received, but the significant and lasting contribution to St. John’s mission.

Please note, when filling out the virtual form, state both the nominee and the achievement for which they are being nominated. If you do not have someone to nominate for a specific award, please respond N/A to that question.

Spiritual Leadership Award (This award recognizes that parishioner who has made a most significant and lasting contribution toward the spiritual growth of our parish and/or school either as a volunteer or as an employee.) *
Excellence in Ministry Award (This award recognizes that parishioner who has made a significant and lasting contribution toward the development and sustainability of one or more of the ministries sponsored by St. John the Evangelist Parish.) *
Excellence in Service Award  (This award recognizes that parishioner or group of parishioners who have made an exceptional contribution toward the overall growth of our parish by consistently engaging in a great number of parish activities.) *
SJE Loyalty Award (This award recognizes that parishioner or family that over an extended period has demonstrated outstanding loyalty and devotion to the mission and identity of the St. John the Evangelist community.) *
