Instructors can use this form to file an "Intent to Report". Submitting this form tells the Academic Integrity Office that you suspect a possible academic integrity violation in your class, but you do not have time immediately to meet with the student or submit a full allegation report. If you file this Intent to Report within 5 business days of detecting a possible violation, the student will be prohibited from withdrawing from the class.
