ONLY A PARENT OR LEGAL GUARDIAN MAY SUBMIT THIS FORM. Submit this form if your child is lawfully absent from virtual learning. Notification must be made within 3 days of the student's absence. It is the student's responsibility to request make-up work from teachers when returning from a lawful absence. Submit only one student's name per form submission.
