This form is to be completed AFTER Volunteer Hours are completed!
For reimbursement of the league volunteer fee, each family must serve a minimum of four (4) hours of volunteer time by completing one or more of the tasks below.
*Please note- registering as a 'team parent' (or other volunteer position) deducts the volunteer fee at the time of registration!

Once completed, please fill out the form in it's entirety and click "Submit".

Once submitted, you should receive a confirmation screen and email.

The league treasurer will make every effort to process your reimbursement within 30 days of submitting the form- reach out to info@arcadecreekll.com with any questions.
