This form is to request a change of subjects, or to drop a 4th subject or enrichment option. Please note that not all combinations are possible, and that you must continue to attend the subject until you have received an email to your school email address confirming the change and your Edulink timetable has been updated.
If you do not continue to attend the subject you are leaving, you will be marked absent and your attendance will be affected.

You must arrange for a parent to email options@calday.co.uk to approve the change request before submitting this form.

You must also discuss the change with your form tutor, the subject teacher for the subject you are leaving, and your head of year before submitting this request. A copy of this form will be sent to these members of staff to formally approve your request if the change is possible and fits within the timetable.

NOTE: due to part time working, any requests submitted on a Monday or Tuesday may have a slight delay in processing
