This confirms that I will provide transportation to and from Rogers International School for my child for the 2023-2024 school year.

I understand that attendance records will be used to calculate reimbursement to me at the rate of $5.00 per day at the end of the school year.

In order to receive this reimbursement:
1. Parents must provide transportation for the entire school year.
2. The student may not reside in the same town as the school.
3. The transportation provided must be from home to the school.
4. Reimbursement to parents with more than one (1) student at the same school is limited to a total of $5.00 per day, per the State Department of Education.
