*ATTENDANCE POLICIES: No refunds or credits will be issued for missed days of club. Payments will process immediately upon registration, reserving your child’s space in the class. If for some reason we are unable to process a payment, you will be notified by email or phone. Your child’s space in the club is not reserved until payment is paid in full. I understand that I will be charged a fee for every time my child is picked up more than 10 minutes after the end of class at a rate of $10 for every 10 minutes.*

*BEHAVIOR EXPECTATIONS: Any student unable to maintain appropriate behavior in class, may result in not being allowed to attend future club dates. Parents will be notified with a written and verbal warning prior to any removal. Refunds/ credits will NOT be issued for any student removed for this reason.

*MEDICAL CONSENT: The undersigned gives permission to Ex Animo Creations, its owners, and employees, to seek emergency medical treatment for the participant(s) in the event they are not able to reach a parent or guardian. I hereby declare any physical/ mental problems, restrictions, or condition and/or declare the participant(s) to be in good physical and mental health.*

*RELEASE STATEMENT: As the legal parent or guardian, I indemnify and keep harmless Ex Animo Creations, its owners, and employees, against any and all liability, claims, judgements, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Ex Animo Creations, its owners, and employees or in route to or from any of said premises. Ex Animo Creations is not responsible for the supervision of participant(s) more than 10 minutes before or 15 minutes after the participant’s scheduled class times. Ex Animo Creations reserves the right to photograph and take video of classes, performances, and participants for training, advertising, or any other written permission.
