Use this form to apply for the Fort Atkinson Wrestling Club Inc. Scholarship. Answer all questions below and submit electronically.  All applications will be reviewed by the FAWC Board and award recipients will be voted on by such board.  Winners will be announced at the Fort Atkinson High School Scholarship Awards Banquet.  Please note that the number of awards and monetary value of the awards will be determined by the Board based on number of applications and eligible applicants.  Scholarship reward(s) will not be less than $250 and not exceed $500.

If selected to receive the award, you must show proof of enrollment before any monetary exchange is made.

Eligibility Requirements:
Fort Atkinson High School senior.
Must have participated in the High School wrestling program for no less than a total of 2 years.  
Must have a cumulative GPA of at least 2.5.
Must be used at an accredited university/college or technical school.
Former students are not eligible.

Due date for this scholarship will be March 1st.
