Document Copy(s) Request Form (DCRF)
Use this form to request a copy of documents recorded by the Clay County Clerk's Office in West Virginia. 

Step 1: Input all relevant information below, 
Step 2: Submit this form for processing, 
Step 3: Wait for a response from the County Clerk letting you know of the fee for the requested document, and
Step 4: Upon receiving a correspondence from the County Clerk visit: https://www.claycountywv.us/agencies/county-clerk follow these steps:
  1. Click on and navigate to Agencies > County Clerk,
  2. Select "Make a Payment" ,
  3. Input account number 7416945 when prompted to do so, 
  4. Send an emailing confirming your payment has been processed to county.clerk@claycountywv.us
