Ms. Wallace: Counselor for 7th grade (last names N-Z) and 8th grade
Students: You may use this form to refer yourself to the counselor. Once you submit this form, your counselor will contact you.
School Personnel (teachers, admin, etc): You may use this form to refer a student to the counselor that you may be concerned about. We will reach out to the student and go from there. However, if it is an emergency situation, please also CALL your counselor immediately.
Parents: You may use this form to refer your child to the school counselor. You can also call us at school if you would rather. 919-775-7351. Ms. Wallace 'sext. is 1186, Mrs. White's extension is 1106. Once a counseling request has been received, we will respond as soon as possible. This form is confidential. Your submissions will only be seen by the counselors.

**Important Note** Responses to this form are checked when school is OPEN. If school is closed (after school hours, weekends, holidays, summer, etc.), the responses will not be seen until school is open again. If you are a student and this is an emergency, please seek the help of a trusted adult.
