We strongly encourage you to submit OSD exam accommodation requests as early as possible and respectfully require a minimum of 3 business days advance notice to arrange all Biology OSD exam/quiz accommodations.

If you will have in-person exams, you need to fill out the form below for each biology course you are enrolled in this quarter. Please list ALL in-class quizzes, midterms, and the final exam. The exam/quiz times you request will need to match the dates/times posted on each course syllabus.

Once you fill out this form, the Biology OSD Liaison will verify the information and, if appropriate, will process your request and send a confirmation email to you (the student) and the course instructor.
