This consent form is for participation in tests designed to detect asymptomatic coronavirus cases.  It has been taken directly from the Government.

This form is designed to collect consent (or not) to undertake regular testing in line with the government national testing programme for school children.

Please could you indicate if you give consent for this to happen as soon as possible.

Be aware the DfE and the NHS have advised us that “people who decline to participate in serial testing will follow the usual national guidelines and are legally obliged to self-isolate following contact with a positive case of Covid-19 according to the advice given to them by the NHS Test and Trace service”.

Consent relates to the following groups of students and staff as follows:

Students younger than 16 years - this form must be completed by the parent or legal guardian. Please complete one consent form for each child you wish to participate in testing.

Students aged 16 or over who are able to provide informed consent - can complete this form themselves, having discussed participation with their parent/guardian if under 18.

For any student who does not have the capacity to provide informed consent- this form must be completed by the parent or legal guardian. Please complete one consent form for each child you wish to participate in testing.

Staff will complete this form themselves.
