If you have an idea for a social or cultural event such as a faculty lecture, movie night, or academic enrichment workshop, complete this form to provide the necessary information to the Office of Student Life.

After completing the form, schedule an appointment with associate director of student life Anthony Sweeney (anthony.sweeney@cuny.edu) who will work with you to develop your ideas into meaningful programming for you and your classmates.

All activities and events are subject to approval by the Office of Student Services.
