Students who need a new ID because it is lost, stolen, or broken should select "Need new ID printed" as the reason.

If you have a working ID but left it at home or don't have it with you, do not select Need new ID printed.  select "Forgot ID".

If you have already requested a new ID but have not yet received it, select "Waiting for ID - already requested".
