Complete this form to register your children for Happy Days Club Wrap-around Care. Before you get started, please note the following to make sure that you understand the form and have everything that you need to hand:

👨‍👩‍👧‍👦 We'll ask you to tell us about yourself, your child and anyone who you want to be linked to your account, either to contact in the event of an emergency, or collect your child from Happy Days, so please have the details of anyone else that you want to tell us about to hand.

📆 We'll ask you to tell us the days you'd like us to take care of your children, but don't worry if you're unsure or you think this could change. Unlike other providers, we're really flexible and even offer an ad-hoc booking service if your childcare needs change frequently, or you need book extra sessions (you can even book the night before if there's space).

⏳The form should take approximately 5 minutes to complete (a little longer if you are adding multiple contacts or nominating people who can collect your children).

✍️ Ticking the boxes and submitting this secure online form is providing us with your consent for your child to attend Happy Days Club and tells us that you have read and understood all of the terms by which your child will be cared for. Please make sure that you take the time to read these, so that you understand our obligations to you and yours to us.
