Global Arts Live Performance Enrollment Form 2024
Symphony Space looks forward to hosting your school at one or more of our Live Performances this year! Schools may attend performances in-person or via livestream at the dates and times listed below.

In-Person Performance Schedule:

Native American Music, Dance and Storytelling

Latin American Music, Dance and Storytelling

Asian Music, Dance and Storytelling
  • Wednesday, May 22nd at 11:00AM - Limited seats available

Livestream Performance Schedule:

All interactive cultural performances run 11:00 AM-12:15 PM ET.

Please fill out a new form for each performance you wish to attend. This Enrollment Form functions as a contract between your school and Symphony Space.

Pricing
We offer different admission rates based on how your school will attend:
  • In-Person Performance at Symphony Space: $7 per ticket, plus 1 FREE chaperone ticket for every 10 students
  • Livestream the performance to your school: FREE for Title 1 Schools or $20 per class
All participating schools will receive a Resource Guide with pre- and post-performance activities in the weeks before the performance.

Payment Details:
  • Payment may be made by check or DOE Purchase Order. 
  • An invoice will be sent via email within 3 business days of submitting this Enrollment Form. 
  • This Enrollment Form will hold your seats, however your reservation will not be complete until we've received payment by check, or a Purchase Order number from the NYC DOE.
  • If paying by check, your receipt will be sent once Enrollment Forms and payment are received. Payment by check must be received one month prior to performance to complete and confirm your reservation.
  • If by paying by purchase order, your receipt will be sent once Enrollment Forms and DOE Purchase Order numbers are received. Schools must certify and send class rosters to NYC DOE upon completion of Cultural Interactive Performance.

Cancellation Policy

By submitting this registration form, you also agree to the following cancellation policies:

A. If any scheduled field trip visit is cancelled by the school with less than 4 weeks’ notice, the school will forfeit 50% of total cost per cancelled field trip.

B. If a school group, for whatever reason, does not attend the scheduled field trip date, Symphony Space shall be under no obligation to refund payments. 

C. Symphony Space shall not be responsible for events beyond its control, such as, without limitation, acts of God, strikes, acts of war, terrorism or civil disturbance and government restrictions. If any scheduled field trip is cancelled because of one of these events, schools will be reimbursed 100%. We will offer an alternate date if possible.

D. Snow days are rescheduled and occur in May.
