By signing this form, you agree to allow your student to participate in the above mentioned activity.  Due to current pandemic situations, masks may be required to participate.  Starstruck Studios Dance & Tumbling will abide by State and Local Health Laws at all times.  Our first priority is to the health and safety of everyone that participates and works with Starstruck Studios. By signing, you understand the inherent risks and choose to allow your student to participate of your own free will.  I agree to indemnify and hold harmless Starstruck studios its successor and/or assigns from all liability, claims, demands, losses, or damages on minors account caused or alleged to have been caused in whole or in part by negligence or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minors behalf makes a claim against Starstruck Studios, its successors and or assigns, I WILL INDEMNIFY AND HOLD HARMLESS from any litigation expenses, attorney fees, loss, liability, damage, or cost Starstruck Studios may incur as the result of any such claim. I understand that there will be no refunds given or make-up lessons scheduled for any classes missed by student(s).  I further understand and do hereby grant permission to Starstruck Studios LLC to use the image(s) of my child(ren) listed above.  Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the Starstruck Studios website.  I agree that these images may be used by Starstruck Studios for a variety of purposes and that these images may be used without further notifying me.  I do understand that the child's last name will not be used in conjunction with any video or digital images for marketing purposes.  No remuneration will be given to any participant for images used or as referenced above. By signing this Release/Authorization I attest that I have read and understand the above terms of this waiver, and agree to abide fully by its terms.
