This form is for:
1. Reserving a registration spot in the class, as it does NOT guarantee your child will qualify for participation;
2. Obtaining your expressed, informed consent to the terms & waivers for participation;
All other requirements set forth in the information packet must be met to complete registration.
Applicants must attend and pass ONE of the TWO scheduled swim tests and pay for the class.
DATES FOR SWIM TESTS: SATURDAY MAY 11 AND SATURDAY MAY 18; 9:00 AM ED WALLINE REGIONAL BEACH ACCESS
APPLICANTS AND PARENTS MUST ATTEND ONLY ONE OF THE TWO, BUT A PARENT AND APPLICANT MUST BE TOGETHER AT THE CHOSEN SWIM TEST DATE TO COMPLETE REGISTRATION AND SECURE A SPOT.
Once the class registration is filled per session, those who enter these forms will be put on the waiting list, moved into spots as they open up before & after the swim test dates, and up to the beginning of classes.
We recommend that you submit this form even if the registration is already full for the session you want most.
The first 30 registered applicants per session who pass their swim test and submit payment will become participants in the SWFD Junior Lifeguard Program.
