USE THIS FORM TO:
• Apply for Lesson and/or Music Camp Scholarships.
• Access your Student Account Funds.
• Request Matching Funds for Instrument Purchases.
ALL FUNDING REQUESTS - The Music Boosters will review all submissions quarterly with funding decisions made and presented in September, December, March, and June. In order to be considered for funds for the upcoming quarter, you will need to fill this form out BEFORE the Music Booster meeting on each of these months (currently the first or second Wednesday of the month).
SCHOLARSHIPS - The MDI Music Boosters will award Music scholarships quarterly to Music students in the MDI school system between grades 5 and 12 to help cover private music lessons or music camps. Scholarships may be requested even if camp/instructor acceptance has not been confirmed. Music Camp funds will be released upon confirmation of acceptance. The Music Boosters can pay instructors/camps directly or reimburse you as may be appropriate.
STUDENT ACCOUNTS - High School Students may make withdrawals from their student accounts to cover music related expenses. Some examples of music expenses that the student accounts cover are: private lessons, concert tickets, travel related to participating in music events like Jazz Festival or Show Choir festivals, purchase of music equipment or related supplies. If you have questions about what might be covered, your music teacher can approve other related music activity expenses.
MATCHING FUNDS - The Music Boosters will provide matching funds for High School student instrument purchases. We require that the students be in High School and they must contribute an equal amount of funds from their own student Music Booster accounts. The Music Boosters will match the student contributions up to a total of $1000 per student (over the student's 4 years of High School, not per purchase).
