Education Outside The Classroom Activities (EOTC) – Permission Form.

Each year your child will be involved in a number of EOTC activities.  These activities provide learning opportunities in various environments.

Examples of EOTC activities your child could be involved in are trips to local farms, town library, beach trip, sporting events, swimming pool, local schools, gardens and Leadership Days.

This form will give permission for your child to leave our school to participate in the type of activities listed above.  All EOTC activities are planned and parents will be given details of the event, date, transport arrangements, costs, request for help with supervision etc.  

A simple walking trip off-site within the CBD will be treated as a normal class activity.

Parents will always be informed of an EOTC event through a newsletter, phone, Hero (linced), website or email.

This permission slip is valid for the duration of your child's enrollment.  One form per child needs to be completed and submitted.

Please note:  Overnight EOTC activities will require separate permission signed by parents. All car/van transport vehicles will have a current warrant of fitness, registration and the driver will have a full licence. Under New Zealand law, all children under seven years of age must use an approved child restraint appropriate for their age and size. Children aged seven must be secured in a restraint if one is available in the vehicle. It is your responsibility to supply this to the classroom teacher if this is applicable to your child.

Please submit this form as soon as possible. By submitting the form you are giving permission for your child to attend any EOTC activities held during a school day and during there duration of schooling at Cambridge Primary School.
