2020 TIS Middle and Secondary Summer School Registration Form
Please complete this form ONCE FOR EACH STUDENT you wish to register.

Placement is made on a first come, first served basis until the maximum number of students in the course has been reached.

Payments will be accepted at the TIS NW office from June 10 - 24 during regular office hours. Please make sure to come in and pay for your sessions during that time.

To calculate your payment, multiply the total number of weeks by:
TIS STUDENT:   $1500 /week/ session                  
REGULAR:          $2200 /week/ session

We reserve the right to cancel any course that does not meet the minimum enrollment required. In the case of cancellation you will receive a full refund.

This form will send you a summary of your responses.

Each course is a HALF DAY session, either Morning AM or Afternoon PM
