The purpose of this survey is to help the CFHS athletics office track students' athletic physicals and anticipate which students will be participating in each sport. Per AIA guidelines surrounding COVID-19, students that have a 2019-2020 physical on file do not need a new physical unless they do not meet the AIA health/injury qualifications (see "2020-2021 Athletic Physical Requirements" document on CFHS athletics webpage for specifics). However, all returning athletes will still need to fill out CFHS Athletics Registration Form, AIA Concussion Form, and Consent to Treat Form. All incoming freshman, newly enrolled CFHS students, and returning students that do no have a physical on file from last school year will need to submit a completed physical packet to Mrs. Welker in House 4. The packets and forms can be picked up from House 4 or printed from the CFHS athletics webpage.
