In line with Government guidance please only use this facility if you have no other means of childcare and you are a key worker on the front line as described on the government's list of key workers. If you have a partner who is not on the key worker list please consider if you really require this extraordinary cover.

You would also need to fill out this form if your child fits within the vulnerable groups description as described on the department of education website.

Please can we ask that you now complete this form as many shift workers will not require cover every day.

I realise it's a pain but could you fill out one form for each child to enable us to provide the correct staffing for the age of the children.
