This form is used to apply to be considered for the PNW Key Club Club District Project Award for the 2020-2021 term.

The Club District Project Award shall provide recognition to individual Key Clubs for exhibiting outstanding fundraising for the district project, the Thirst Project, in a single year.

ELIGIBILITY
Any active PNW Key Club in good standing that submits a completed Annual Achievement Report. The activities covered by the reports may only include those undertaken between April 1, 2020, and February 1, 2021.

CRITERIA
A. Clubs shall compete with other clubs of similar size within four membership categories based upon membership from the previous year:
Bronze: 35 members or less
Silver: 36-60 members
Gold: 61-85 members
Platinum: 86 members or more
B. The club with the most money fundraised in each of the membership categories shall be deemed the first place winner.
