Support your vulnerable patients at risk of COVID-19.

As part of a pilot project, Darling Downs and West Moreton PHN is offering 20 Darling Downs and West Moreton Practices access to GoShare Plus and SMS credits to enable them to send high-risk patients information about COVID-19.

GoShare Plus is available through CAT4. It enables you to identify high risk patients using a CAT4 recipe, and send them one of four information packages directly to their mobile phone

The interactive information packages available are:
1. How to minimise the risk of COVID-19 infection (11 languages)
2. How to minimise the risk of COVID-19 infection (Aboriginal and Torres Strait Islanders)
3. What to do if you have symptoms of COVID-19 (11 languages)
4. What to do if you are diagnosed with COVID-19 (currently English only)

If you are one of the first 20 practices, after completing this registration form, Healthily will activate your GoShare Plus license and will contact you to allocate SMS credits and provide support to send the information packages to your patients. If the 20 funded licences have already been allocated we will contact you and place you on a waiting list for a license should this project be extended.
